Covenant Benefits, PLLC collects information from you required both for our business and pursuant to regulatory requirements. Without it, we cannot provide our products and services for you. We will be collecting protected information about you from:

We collect a variety of personal information to administer a member’s medical or ancillary coverage. Some of this information is provided by members in enrollment forms, surveys and correspondence. We also receive personal information (such as eligibility and claims information) through transactions with our affiliates and members, employers, insurance agents, other insurers, and health care providers. We retain this information after a member’s coverage ends. We limit the collection of personal information to that which is necessary to administer our business, provide quality service and meet regulatory requirements.

The information we gather is disclosed within our company to help us maximize the services we can provide to our customers. We will only disclose your protected information as is necessary for us to provide the insurance products and services you expect from us. We do not sell your protected information or customer lists to third parties.

Covenant Benefits, PLLC is not responsible for the information privacy practices or the content of such web sites and we urge you to view the policies of these sites before providing personal information.

In addition, we may disclose your protected information to medical care institutions or medical professionals, insurance regulatory authorities, law enforcement or other government authorities, or to affiliated or nonaffiliated third parties as is reasonably necessary to conduct our business or as otherwise permitted by law.

We treat personal information securely and confidentially. We limit access to personal information to only those persons who need to know that information to provide our products or services to members. These persons are trained on the importance of safeguarding this information and must comply with our procedures and applicable law. We meet strict physical, electronic and procedural security standards to protect personal information and maintain internal procedures to promote the integrity and accuracy of that information.

You have the right to review the protected information about you relating to any insurance product issued by us that we could reasonably locate and retrieve. You may also request that we correct, amend or delete any inaccurate information by requesting the correction in writing to us.
