We collect a variety of personal information to administer a member’s medical or ancillary coverage. Some of this information is provided by members in enrollment forms, surveys and correspondence. We also receive personal information (such as eligibility and claims information) through transactions with our affiliates and members, employers, insurance agents, other insurers, and health care providers. We retain this information after a member’s coverage ends. We limit the collection of personal information to that which is necessary to administer our business, provide quality service and meet regulatory requirements.

The information we gather is disclosed within our company to help us maximize the services we can provide to our customers. We will only disclose your protected information as is necessary for us to provide the insurance products and services you expect from us. We do not sell your protected information or customer lists to third parties.

These persons are trained on the importance of safeguarding this information and must comply with our procedures and applicable law. You have the right to review the protected information about you relating to any insurance product issued by us that we could reasonably locate and retrieve. You may also request that we correct, amend or delete any inaccurate information by requesting the correction in writing to us.
